As the East Regional Clinical Performance Nurse at Monogram Health, I supported a team of clinical staff in 10 territories to provide effective care management for high-risk patients with chronic kidney disease (CKD), end-stage renal disease (ESRD) and multiple comorbidities. I have over 34 years of nursing experience in various clinical and leadership roles, and hold a both a care management certification (CCM) from the Commission for Case Management Certification (CCMC) and a critical care registered nurse (adult) credential (CCRN-A) from the American Association of Critical-Care Nurses. My passion is to innovate and implement processes that improve the quality of patient care and maximize the productivity of our field and telephonic care managers. I developed and led a Transitional Care Management pilot that reduced admission/readmission rate in Florida from 35.3% to 16.9% in just 30 days. I also prepared and led the daily regional meetings with Regional Medical and Operations Directors to ensure optimal clinical management of high-risk patients. Additionally, I provided continuous education to clinical staff across the country on disease process and management, post-hospital discharges, value-based care, population health, and quality measures.
